Contract Between

HealthWise Monthly & Delivery Person

1. You must commit to at least 6 consecutive months of delivery to enter into this agreement. This would be delivery once a month for six times.

2. The papers are generally printed around the 1^st of each month. We absolutely expect that they will be delivered within 2 days of when you are contacted for pickup.

3. You are responsible for any parking tickets, accidents etc. that occur while delivering the papers.

4. The papers must be unbundled when they are delivered. Do not leave any bundled papers at a delivery location.

5. The routes can vary from month to month with the addition and subtraction of delivery spots.

6. Routes usually take between 2-3 hours a month to complete.

7. Papers are picked up in Muskego or Cedarburg by you unless otherwise arranged.

8. The size of the ad you will get is 2.25w x 3h, black and white.

9. We have a second paper called Twenty Something that is published 4 times a year that will be delivered to some of the same spots that you deliver HealthWise Monthly. In return, you will receive an ad in that paper as well.

These papers are to be delivered on our schedule and not at your convenience. Please respect this.

Delivery is crucial to our business and if you are not willing to treat it with respect please do not get involved. We are not interested in chasing you down to get the papers delivered.

You have to make time in advance to deliver the papers in a timely fashion. This is a deal where you are getting advertising for delivery and we expect you to fulfill your part of the commitment.

IMPORTANT

The papers are delivered the 1^st of each month. We expect that all papers be delivered no later than the 3^rd, unless otherwise instructed. We expect the papers when delivered are delivered to ALL locations on the list we provide, unbundled in a neat fashion.

We expect you to fulfill all that is instructed. (If not, we will charge you for any advertising that is published) The value of the ad you are receiving is $70.

EMAIL

For anyone who has an email account, we will contact you primarily through email. Please respond promptly to all email inquiries. We will generally send you an email letting you know when the papers are coming in and an email afterwards asking how the delivery went. It is important that we get this information to keep improving our routes.
